Plan types & coverage. castlehealthplans.com offers multiple plan options. Some options are health insurance / major medical and some options may include non-insurance membership services (such as telehealth, pharmacy support, or other partner-delivered benefits). Your exact benefits depend on the plan you enroll in and the plan documents you receive at enrollment.
ACA / Minimum Essential Coverage (MEC). Gold, Gold +, and Platinum are intended to provide Affordable Care Act (ACA) Minimum Essential Coverage (MEC) (as described in the plan documents). Always refer to your Certificate of Coverage / Summary of Benefits for the official terms, covered services, and limitations.
Major Medical (Gold +). Gold + is a major medical insurance option and is designed to include benefits for hospitalization, emergency room (ER) services, and surgery, subject to the plan's deductibles, copays/coinsurance, prior authorization rules (where applicable), network requirements, exclusions, and benefit limits. Coverage varies by plan and state; not every provider, facility, or procedure may be covered.
Emergency / 911. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
Providers are independent. Telehealth, clinical services, facilities, and other care are provided by independent, third-party licensed clinicians, medical groups, and partner networks. Clinical decisions are made solely by the treating provider.
Costs & billing. Your out-of-pocket responsibility may include deductibles, copays, coinsurance, non-covered services, and charges from third parties. Participating providers/facilities may bill separately for services not included in membership pricing, out-of-network services, or items not covered by the plan's terms.
Prescriptions not guaranteed. Medication is prescribed only when clinically appropriate and at the provider's discretion. Formularies, coverage tiers, quantities, and delivery options vary. Partner pharmacy rules and state laws may apply.
Availability varies by state and can change. Program features, partner networks, covered services, and shipping/delivery availability may vary by location and are subject to change without notice.
Privacy & data. Partner providers and pharmacies maintain their own compliance and privacy programs, and your information may be shared with them as necessary to deliver services. Please review our Privacy Policy and partner notices for details.
Eligibility. Primary member must be 18+ (or as required by state law). Adding dependents and household members is subject to plan rules and enrollment requirements.
